Form 21-0781 · In-service traumatic event statement

Mental Health Statement Drafter

You write the memory in your own words. We turn it into the structured statement raters look for under M21-1: specific date or date range, unit and role, the event itself, and how it shows up in your daily life now. MST-aware (38 CFR 3.304(f)(5)) — if you don’t have a corroborating witness, we surface the markers in your input that count.

This is not only a PTSD form. Since March 2024, VA Form 21-0781 covers any mental health condition you connect to a traumatic event in service — depression, anxiety, and bipolar disorder as well as PTSD. If you were told to file 21-0781a for a personal assault or MST claim, VA discontinued that form on June 28, 2024 and moved those claims here. Use your own diagnosis below; you do not need a PTSD diagnosis to use this form.
